I bought this prototype from Matt Martin (nesman85) along with a few others. Matt got them from a former video game journalist who lives right near him. The label just has the title and three numbers most likely the date just like my Tale Spin prototype that came from the same source.

Also like Tale Spin this prototype is quite a bit different from the released version. More about this in a future update. :)
